Prendergast Ladywell School is an all-through school for children aged 4 - 16 years. This form is for children applying to Year 7 - Year 11. (Year 7 in-year applications are only accepted after 1 January. Please contact Lewisham Admissions until 31 December.)

This application form must be completed by a parent/carer with parental responsibility to apply for an in-year place at Prendergast Ladywell School. We may require the applicant to provide formal evidence as proof of parental responsibility.
You must provide the following documents to complete your application:
- Proof of address where child normally lives includes: current council tax statement/letter (current year); tenancy agreement (current); confirmation of house purchase; letter from the National Asylum Support Service (NASS) or UK Border Force (UKBA). If this is a new address, you must also supply a letter from a solicitor confirming completion date (closing accounts may be required). If you move house, or some other relevant change to your situation, you must re-apply and provide new documents.
- Proof of birth date includes: child’s current passport; NHS prescription or medical letter (recently dated and showing child's address and DOB).
